where to turn,

beware of long-term use of steroids - they may irreversibly thin your skin. It sounds to me, that hemorrhoids may be an issue again, and they probably result from repeating strain. Lazy colon usually result in severe constipation, so I'm not sure if it is this?

Are you male or female, how old are you?
What's your diet in general?

Being physically active (walking can help), drinking sufficient amount of water, eating foods rich in fiber, and changing constipating medications (if any) can prevent constipation.

I have had itching for years - first it was definitely hemmerhoids, and itching round the clock. That seems to have gone away, but I have gradually (at least 3 years!) intensifying itching when I need to make a bowel movement or gas. Relief comes with itching and going, though I have started straining for tiny bowel movements several times a day, and one healthy movement.

Doctors have recommended steroids, which seem to work while I am using them.
Could it be lazy colon? Any suggestions?

To Sore and fed-up.

Methadone can cause severe constipation. It is possible this is a major cause of his problems. Fissure has to be treated and has to be treated well whatever it takes.

Hi there; my husband has had many health issues over the years. Seven years ago he fell off the roof, fracturing his c7-T1, sternum and acouple lumbars. He was prescribed Oxycodone, and got addicted. He was (and is) on methadone to get him off the oxycodone/contin. He also takes CPZ, gabapentin, effexor. He has had to give himself an enema to have a BM for the last 7 years. He has been refered to many doctors who have tried different things. He has had a colonoscopy, CT scan, another one tried botox to relieve the tightness. He saw a surgeon in Dec., who gave him a sphincterotomy, then banded his internal hemorrhoids, then surgery to remove them. He is still in a lot of pain, not able to work the last 2 years. Now the surgeon says its a fissure and wants to do another surgery. He has tried various creams to no avail. His family GP says to have no more surgeries, but doesn't know where to go from here. What can we do? Life is so unpleasent for him and is really affecting family relations. Any Advice? Thanks in advance for your help.

Hi Jan,

I went to China in last year in May and got severe diarrhea while I was there. A couple weeks after that and everyday since then, I've had anal itching. I've thrown everything but the kitchen sink at it and I've been to three different doctors. Here is the treatment I've tried so far:

Date: May 2009
Provider: Primary Care
Ova and Parasites exam (negative)

Date: June 2009
Provider: Gastroenterology
Prescribed: Analpram (Hydrocortisone Acetate 2.5%, Pramoxine HCl 1%), switched to baby wipes,
epsom salt baths

Date: Early July 2009
Provider: Gastroenterology
Prescribed: Lotrimin, Triamcinolone 0.1%, Clobetasol .05%

Date: Late July 2009
Provider: Gastroenterology
Procedure: Sidmioscopy - no problems identified, recommended Desitin OTC

Date: August 2009
Provider: Dermatology
Prescribed: Steriod Desoximetasone 0.25%, Fluconazole 200 MG antifungal, and pinworm treatment
(OTC)

Date: September 2009
Provider: Dermatology
Procedure: Skin biopsy (negative for anal cancer and psoriasis)

Date: October 2009
Provider: Dermatology
Prescribed: Steriod Desoximetasone 0.25%, Preparation H Suppositories

Date: November 2009
Provider: Dermatology
Prescribed: Protopic 0.1%, Zonalon 5%

Date: December 2009 – January 2010
Treatment: switching between Desoximetasone 0.25% and Desitin OTC

Date: January 2010
Provider: Primary Care
Recommended: cocoa butter or mink oil

Date: February 2010
Provider: Primary Care
Recommended: continue cocoa butter, add Solarcaine (OTC)

Unfortunately none of this has solved the problem and I still have irritation in the area. I even went so far as to install a bidet in my house so I no longer need to use wipes..I just use TP to gently dry off afterwards. I do have a lot of hair down there, and there is some redness. I never scratch the area, but I can't even walk without it causing irritation.

I'm at a loss as to what to do next. Thanks for any help.

Clark,

does he experience urgency to have a bowel movement or other digestive problems or any other symptom (fever, pain...)

Redness with an obvious rash or scaling, or any other changes on the skin surface in the groin area can be from:
- a fungal infection (jock itch). An antifungal ointment can help.
- ecczema from allergic reaction, for example, to pants

Reddened, slightly raised patch, without other surface changes could be:
- hives
- insect bite
- cellulitis - a strep or staph infection of the underskin tissue, possibly from a smal accidental injury (cut, scratch) in the groin
- spread of an infection or inflamation (Crohn's disease?) from the rectum

My bf started out with some redness on the skin on the outside of his sphincter. Now a couple of days later it is bright red, and looks like a major chemical burn.
When it had hurt originally, he applied Preparation H to cool it down, even though he doesn't have a hemmoroid. No itching, just very painful.
What caused this? And how do we fix it? Has anyone been through this before? I cannot seem to find any advice pertinent to his situation. And no he is not gay, as I AM female. Thanks for your help.

To frogman.

That lump could be a hidradenitis suppurativa - a cluster of inflammed oil and sweat glands that can be infected by bacteria (staphylococci). I recommend you to visit a doctor when that lump appears, so doctor can see what is it.

To judra36.

In herpes, a group of small translucent blisters surrounded by red skin are typical. A shallow ulcer may occur, but cracks are not typical. They may itch, burn or cause pain. They are spread by sexual contact, but not necessary as a new infection. Herpes viruses may stay in the body for several years and outbreak during periods of stress or infections, like flu. Not sure if contraceptive pills have something with it. Herpes can be spread even if affected person has no visible rash. Autoinfection with hands (from mouth herpes) is also possible.

In folliculitis, red bumps appear one by one, not in groups, and often have a white pus-filled center. OTC antibiotic ointment mupirocin helps.

Cracks may be from scratching, or from fungal infection (jock itch).
